Do they? Yes they do. I have built a lever device that attaches to the lower frame of the keyboard and allows the shift key to be operated with the thumb reaching down from the spacebar. I do not build or sell these but most people could have a friend or relative build one for them. It is easy.
I have a keyboard where I cut the lower left Alt key connection to the printed circuit board and wired it across the shift key. It works great. It is not my IBM which I like the feel of so much.
I have wired a foot switch across the shift key. That is a great solution. Probably the best solution for most people is a foot switch. You can buy a foot switch at the music store for about $7.50. Any electronics person can connect it. I don't like the foot switch. I have too much junk around now.
Word processors will capitalize for you. That does not include proper names. It is a solution but a bit more trouble than other ways.
